Comments:
An unannounced renewal study was conducted today. At LI arrival there were 4 children (13 points) with one caregiver. Provider returned within 15 minutes. Children were observed resting and playing, and during a diaper change. 4 children and 6 adult records were reviewed. See Violation notice for missing items. If you have any questions about this inspection, please call me at 703-479-4678. A Subsidy Health and Safety Inspection was also conducted today. LI was present today from 1:30 PM - 3:00 PM

Standard #: 22VAC40-111-760-A-1
Description: Based on a review of first aid supplies, the 1st aid kit is incomplete.
Evidence:
Missing item - digital thermometer.

Plan of Correction: I will get it right away.

Standard #: 22VAC40-111-100-A
Description: Based on a review of records, one record does not include a physical past 30 days of enrollment.
Evidence:
CHild #2's first date of attendance is 8/16/19. There is no physical on file.

Plan of Correction: I will ask for the physical.

Standard #: 22VAC40-111-60-B
Description: Based on a review of records, children's records are incomplete.
Evidence:
*3 of 4 records reviewed were missing father's information. (Children #1,3,4)
*1st day of attendance, 2nd emergency contact information. (Child #2)

Plan of Correction: I will ask the parent for the information.

Standard #: 22VAC40-111-70-A
Description: Based on a review of records, there is no written acknowledgment of all policies and procedures.
Evidence:
Missing information includes, medication policy, provisions of the emergency preparedness plan. (Children #2,3)

Plan of Correction: I will have the parents sign them by Friday.

Standard #: 22VAC40-111-90-A
Description: Based on a review of records, one child's record is incomplete.
Evidence:
Child #2 does not have documentation of an immunization record. Date of enrollment (8/16/19).

Plan of Correction: I will ask the parent to bring in the shot record.

Standard #: 22VAC40-111-90-C
Description: Based on a review of records, immunizations have not been updated every 6 months.
Evidence:
Child #1/2 record does not include any updated immunizations. Date of Birth 9/20/17.
Date of enrollment: 12/1/2/17.

Plan of Correction: I will ask the parent to bring in the record.

Standard #: 22VAC40-111-690-D
Description: Based on direct observation, the diapering surface is not non-absorbent.
Evidence:
LI observed a diaper change on a fleece blanket on the floor.

Plan of Correction: We will review every policy. Diapers will be changed on a diaper pad.
