1) Child #1's record did not have documentation of an annual update. (date of enrollment - 8/27/18)
2) Child #4's record does not contain documentation of the names of persons other than the custodial parents who are authorized to pick up the child.Plan of Correction: I will complete the annual update for Child #1 and I will get the authorizations for pick up from parents for Child #4
Standard #: 22VAC40-111-500-E Description: Based on observation, the provider failed to ensure that the sandbox was covered when not in use.
Evidence:
The sandbox did not have a fitted cover to be used when the sandbox is not in use.Plan of Correction: I will obtain a solid fitted cover for the sandbox.
