54.1-3408 Provider must be MAT certified to administer prescription medication.

Technical Assistance:
Consultation was given on provider getting MAT certification and the DSS model medication form.

Comments:
An unannounced monitoring inspection was conducted today from 1:15 pm to 3:15pm. There were 7 children (17points) directly supervised by 2 staff. The home, 2 staff records, 3 children?s records, 1 household member?s records, evacuation drills, and emergency supplies were inspected. There was one emergency medication on site today. Children were observed napping. Areas of non-compliance are identified in the Violation Notice. Violations:
Standard #: 22VAC40-111-700-A
Description: Based on record review and interview, the provider did not have a written authorization from the parent to administer medication.
Evidence:
1) Child #2 had a medication on site and there was no documentation of a written authorization from the parent to administer the medication.

Plan of Correction: I will ask the parent to sign it.

Standard #: 22VAC40-111-700-B
Description: Based on record review and interview, the provider did not obtain a physician's signature for long term medication administration.
Evidence:
1) Child #2 had a medication on site for more than 10 days (first day of attendance 08/04/19) and there was no documentation of written authorization from a physician.

Plan of Correction: I will ask the parent to provide it.

Standard #: 22VAC40-111-60-B
Description: Based on record review and interview the provider did not maintain a complete record for each child.
Evidence:
1) Child #3's record did not contain a complete health exam as page 1 was missing.

Plan of Correction: I will ask the parent to complete it.

Standard #: 22VAC40-111-70-A
Description: Based on record review, 1 of 3 children was missing an acknowledgment that the parent has received the family day home's policies and procedures as required by the first day of attendance.
Evidence:
1) Documentation of the parent's acknowledgment for the family day home policies and procedures was missing from child #3's record.

Plan of Correction: I will ask the parent to complete it.

Standard #: 22VAC40-111-220-A
Description: Based on record review and interview, an emergency medication would not be given by a provider who has a current MAT certification.
Evidence:
1) Child #2 has an emergency medication on site and none of the caregivers have a current MAT certification.

Plan of Correction: I will get MAT certified.
