An unannounced monitoring inspection was completed today. The physical plant, 3 caregiver/household member files, 5 children's files, caregiver training, children's activities, emergency procedures, drills and supplies were inspected between 12:30-1:40pm. There were 6 children, totaling 12 points, under the supervision of the Provider. During the inspection, children were observed finishing lunch, enjoying free play and preparing for nap time. The interactions between the children and caregivers were seen to be positive and constructive in nature. Each day the children are in care, they bring their lunches from home. Description: Based on file review, 2 out of 5 children's records do not have additional immunizations every six months up to the age of 2 years.
1. Child #1 is 3 years old. The most recent immunization record on file was updated when the child was 19 months old.
2. Child #2 is 26 months old. The most recent immunization record on file was updated when the child was 19 months old.

Plan of Correction: The Provider will ask the parents today for these.

Standard #: 22VAC40-111-180-A
Description: Based on filer review, 1 caregiver does not have documentation of having completed a TB test/screening within the past two years.
1. Caregiver #1's most recent TB test/screening was completed on 4/3/17.

Plan of Correction: This will be completed again.
