Violations:
Standard #: 22VAC40-111-180-A
Description: Based on a review of caregiver health records, it was determined that one caregiver has not obtained a repeat TB Health Screening within the past 2 years.
Evidence:
The provider's TB screening expired on 8/24/19.

Plan of Correction: I am aware that I need a repeat screening. I was waiting for my new health insurance to take effect. As soon as that happens, I will get my screening done immediately.
