Comments:
The licensing inspector conducted an unannounced monitoring inspection on Thursday, September 5, 2019 between 9:17 am to 11:50 am to determine this family day home compliance with licensing standards. During the inspection, there were observation of nine children in care totaling 25 points with two caregiver present. The licensing inspector reviewed the designated children care areas, bathrooms, kitchen area, and emergency supplies at the family day home to determine compliance with licensing standards. During the inspection, the licensing inspectors completed observations of the children participating in various activities to include engaging in free play, interacting with peers and caregivers, bathroom/handwashing breaks, eating breakfast, and diapering.The children's breakfast consisted of french toast/oatmeal, pineapples, and milk. There were three children's, one caregiver's, one assistant caregiver's, and two household member's records reviewed during this inspection. Standard #: 22VAC40-111-90-A
Description: Based on review of children's record and interview, the provider did not ensure to obtain documentation that the child has been adequately immunized according to the requirements.

Evidence: In Child#4's record (start date: 09/03/19) there was no documentation of immunization record found. Interview with provider confirmed the immunization record was not available for review during this inspection.

Plan of Correction: The provider will get parent to provide by September 9, 2019.

Standard #: 22VAC40-111-600-B
Description: Based on observation, the provider did not ensure infant and toddlers shall spend no more than 30 minutes of consecutive time during waking hours, with the exception of mealtimes, confined in a crib, play pen, high chair or other confining piece of equipment.

Standard #: 22VAC40-111-680-A
Description: Based on observation, the caregiver did not ensure to wash their hands with liquid soap and warm running water after diapering.

Evidence: During the inspection, the Assistant was observed diapering a child in care and placing the child in crib, and putting a Pacifier in another child's mouth prior to hand washing after diapering.

Plan of Correction: The provider reminded assistant of hand washing procedures during inspection.
