Reminded Provider that household members need TB, Sworn Disclosure Statement and Central Registry Search when turning 18 years.

Comments:
An unannounced monitoring inspection was conducted today between the hours of 8:20 am through 9:30. There were a total of 5 children (13 points) observed with 1 caregiver. A total of 3 children's files and 4 staff/household member's files were reviewed.Provider stated that the daycare children do not use the swing set located in the backyard. The children were engaged in the following: toy play, napping, hand washing and snack. There were no violations this date.
