Comments:
An unannounced mandated Renewal inspection was conducted today. There were 5 children present (16 points) with 2 caregivers. Children were observed having lunch, washing hands, and playing with a variety of manipulative toys. Reviewed 2 children's files, 2 assistants' files, 3 resident files, menu, attendance, emergency evacuation log and supplies, and physical plant. Violations:
Standard #: 22VAC40-111-920
Description: Based on observation, the menu for the current one week period did not reflect meals and snacks provided by the family day care home.
Evidence:
The menu for the current day stated meat, vegetable and grain- children were observed eating a pizza, black beans, lettuce and milk.

Plan of Correction: I will update my menu

Standard #: 22VAC40-111-120-A
Description: Based on record review, Assistant caregivers' records did not contain the required information.
Evidence:
Assistant#1 and Assistant#2 were missing Hire date, Address, Verification of age, Emergency Contact address and phone, 2 references prior to hire, documentation of education and training.

Plan of Correction: I will get the paperwork

Standard #: 22VAC40-111-60-B
Description: Based on record review, child's records did not contain required information.
Evidence:
1. Child#1 and Child#2 did not have documentation of parents' work addresses.
2. Child#1 (9 months) did not have permission to administer topical non-prescription skin products. Provider stated the child has diaper cream, and it's administered once today.

Plan of Correction: I will ask the parents for the information.

Standard #: 22VAC40-111-200-A
Description: Based on record review and interview with provider, the provider did not orient the assistants by the end of their first week of assuming the job.
Evidence:
Assistant#1 and Assistant#2 did not have documentation of orientation. Provider stated their start dates were 7/19 for Assistant #1 and in 2015 for Assistant#2.

Plan of Correction: I will train them on my policies
