An unannounced Subsidy Health and Safety inspection was conducted with the vendor on September 23, 2019 beginning at 8:47 AM and ending at 10:57 AM. There were sixteen children in the care of the vendor and three staff members. The children's ages were for a total of 6 months 7 months, 14 months,12 months, 19 months,19 months, 21 months, 23 months, 2, 2, 2, 2, 2, 3, 4, and 4 years old. The children were observed during free play. The children were also observed eating morning snack which consisted of carrot slices, cheese crackers, and milk. Handwashing procedures were observed. Seven children's records were reviewed. Two household members' records and three staff records were reviewed. There were no medications and allergy care plans on site to be reviewed. The first aid kit, flashlight, and weather-band radio were observed and in compliance.

Standard #: 22VAC40-665-230-B Description: Based on a review of 5 staff records, not all of the staff had documentation of completing Virginia Preservice Training for Child Care Staff within the first 90 days of employment . Evidence: Staff A, staff B, staff C did not have documentation of completing the preservice training in their records. Plan of Correction: The vendor will ensure that all staff have completed the Virginia Preservice Training as soon as possible. The documentation will be placed in their records.
Standard #: 22VAC40-665-410-A-3 Description: Based on a review of the emergency drill log and staff interview, the vendor failed to conducted an annual lockdown drill. Evidence: There was no documentation of a lockdown drill being conducted in 2018. Plan of Correction: The vendor stated that she would ensure a lockdown drill would be conducted as soon as possible.
