An unannounced monitoring inspection was conducted today.
Four child were in care. They were resting.
Five children's files and one adult files were reviewed.

Violations:
Standard #: 22VAC40-111-60-B
Description: Based on the review of five children's files the facility failed to update all required information.
Evidence:
1. Enrollment forms for child #1 and 2 do not include insurance information.
2. Enrollment forms for child #3 and 5 do not include actions to take in an emergency for allergies.

Plan of Correction: The provider will ensure that parents complete this information on the enrollment forms.

Standard #: 22VAC40-111-240-A
Description: Based on a tour of the outdoor environment, the facility failed to ensure all areas are clean and safe.
Evidence:
1. There are two ashtrays containing cigarette butts on the porch where the children enter the home.

Plan of Correction: The provider will ensure that the ashtrays are removed.

Standard #: 22VAC40-111-520-A
Description: Based on a tour of the home, the facility failed to provide a crib for each child in care.
Evidence:
1. Child #1 was sleeping in a playpen.

Plan of Correction: The child sleeps in a playpen at home and the provider is working with the family to transition the child from the playpen to a cot.
