Comments:
An unannounced monitoring inspection was conducted today between 12:15 pm and 2:15 pm. There were 3 children (10 points) with 1 caregiver. The children were observed napping, during diaper change and interacting with the caregiver. The physical plant, attendance logs, evacuation logs, bottles, cribs, emergency plan, children's records, caregiver's records, household member's records, and emergency supplies were inspected. Areas of non-compliance were cited on the Violation Notice. If you have any questions, please contact me at Tanya.Johnson1@dss.virginia.gov or call me at 571.423.6392.

Violations:
Standard #: 22VAC40-111-960-E
Description: Based on observation, the provider failed to ensure that bottles are labeled with the child's full name and date.

Evidence:
Child #1's bottles were labeled with the child's name, but did not have a date.
Child #2's bottles were in the refrigerator, but were not labeled with the child's name and the date.

Plan of Correction: I will remind parents to label bottles, and if they don't label bottles, I will label them.
