Comments:
An unannounced renewal inspection was conducted on 10/18/2019 from 9:45 A.M.-12:30 P.M. There were twelve children in care with the provider and two assistants for a total of twenty four points. The records for five children, one provider, two caregivers records, evacuation drills, first aid kit and supplies, emergency plan, chimney inspection, outside play area and required posted information were reviewed. Children were observed during story time, snack time, handwashing, transition to outside play, outside play, painting pumpkins, playdough sensory play, free choice, and departure. Exceptions to compliance are noted on the violation notice.

Standard #: 22VAC40-111-120-B
Description: Based on review of caregiver records, the provider failed to ensure documentation that two or more references as to character and reputation as well as competency were checked before employment.

Evidence:
1. Caregiver #3 only had one reference check in her file. Date of hire was 06/17/2019.
2. The provider verified that there was only one reference check in Caregiver #3's record.

Plan of Correction: The reference check was completed, but there was a change in personnel at the time and most likely the second reference check was misplaced. The provider will complete another reference check and place in the caregiver's file.
