Technical Assistance:
22VAC40-111-520-C Mattresses in cribs are to fit snugly on all sides.
22VAC40-111-530-B Crib mattresses are to be covered with a tight-fitting sheet.

Comments:
An unannounced monitoring inspection was conducted on October 21, 2019 from approximately 10:10 am to 12:50 pm. Provider was present with four children for a total of ten points. Four children's records were reviewed. Provider, household member and assistant/substitute provider records were reviewed.

Violations:
Standard #: 22VAC40-111-520-C
Description: Based on observation, provider did not ensure when cribs are used that the mattress fits snugly.

Evidence: The mattress in the crib curled up away from the corners causing the mattress to not fit snugly to all edges.

Plan of Correction: Provider will try a new mattress today to see if it fits snugly. Provider will also try to see if the fitted sheet can be altered to fit properly.
