Comments:
A mandated monitoring inspection was conducted today. There were 6 children present (14 points) with 3 caregivers. Children were observed playing with a musical guitar, singing songs, bouncing in an exo-saucer, napping, having a diaper change, and washing hands. Reviewed 4 staff files, 1 resident file, 2 children's files, menus, attendance, emergency evacuation plan and log, medications and authorizations, and physical plant. Standard #: 22VAC40-111-700-B
Description: Based on observation, the facility did not obtain written permission to administer prescription medication from the parent or physician.
Evidence:
Child#1 (start date 9/3/19) did not have signed parent or physician permission to administer emergency medication/allergy plan,

Plan of Correction: I will get the parent and physician signature on all forms.

Standard #: 22VAC40-111-920
Description: Based on observation, the menu for the current one-week period was not updated to reflect substitution of foods.
Evidence:
The menu posted indicated the meal for the day was macaroni and cheese, peas. The meal served was chili with beans, macaroni and green peppers.

Plan of Correction: I will make sure to update the menu.

Standard #: 22VAC40-111-70-A
Description: Based on observation, before the children's first day, parent's were not notifies of the family day care home's policies for the administration of medications.
Evidence:
The signed policy for all children enrolled stated the home will only administer non-prescription medication. Provider stated there is a child present with prescription emergency medication.

Plan of Correction: I will have the parent's sign a new form with the correct information.
