Comments:
An unannounced renewal inspection was conducted from 10:30 am - 12:30 pm with the provider. There were five children in care, ranging from 19 months to four years of age, for a total of 11 points. The children were observed working on puzzles, reading books, painting pictures and singing songs. Three children's records were reviewed. Four caregiver/household member records were reviewed. Required information was posted. Annual training hours were in compliance. The emergency drill log was reviewed and in compliance. The provider is not Medication Administration Training (MAT) certified and cannot administer medicine to children in care. Standard #: 22VAC40-111-60-A
Description: Based on review of three children's records, and provider interview, it was determined that the provider did not have all required information in each child's record. Evidence: the record for child #1 (date of first attendance: 10/21/19) did not contain the name, address and telephone number for two designated persons to contact in case of an emergency if the parent cannot be reached.

Plan of Correction: Will get the missing information from the parents.

Standard #: 22VAC40-111-180-A
Description: Based on review of four provider/household member records, it was determined that not all providers/household members had a repeat Tuberculosis (TB) test/screening at least every two years from the date of the initial screening or testing. Evidence: the TB test/screening for provider A was dated 7/11/19.

Plan of Correction: A repeat TB test/screening was conducted. Will locate missing paperwork.
