Comments:
An unannounced renewal inspection was completed on October 22, 2019 from approximately 12:45 p.m. to 2:30 p.m. Eight children, ages infant through four years old, were in care, with two caregivers, for a total of twenty-five points. Caregivers and children were observed in the indoor space engaged in activities to include activities for infants and nap time. Interviews and interactions were conducted and no medications are given. Five children?s records, two caregiver records and one household member record were reviewed. The provider was available for the inspection and was present at the exit interview at which time inspection findings were reviewed and an Acknowledgement of Inspection form was signed and left. Violations:
Standard #: 22VAC40-111-960-E
Description: Based on observation, the licensee failed to ensure that bottles were labeled with the child?s name and date if more than one infant is in care.
Evidence: 1. Several different types of bottles were observed on the kitchen counter with no names or dates labeled on them. 2. Caregiver #1 acknowledged the bottles were not labeled with the date or child?s name as required.

Plan of Correction: The provider labeled the bottles on site and therefore this was corrected.

Standard #: 22VAC40-111-100-A
Description: Based on a review of children?s records, the licensee failed to obtain documentation of a physical examination prior to the child?s attendance or within 30 days after the first day of attendance for two of five children.
Evidence: 1. Child #2's record, enrolled on November 5, 2018, contained a physical dated May 15, 2019. Child #3's record, enrolled on August 5, 2019, contained no physical. 2. Caregiver #1 acknowledged these physicals were not obtained prior to attendance or within 30 days after the first day of attendance.

Plan of Correction: The provider contacted the parent of Child #3 to inquire about the physical. The parent will take the child to obtain a physical on 10/26/2019.

Standard #: 22VAC40-111-120-B
Description: Based on a review of caregiver records and interview, the licensee failed to ensure caregiver records contain all the required elements for one of two caregivers.
Evidence: 1. Caregiver #2's record, hired on October 18, 2018, lacked documentation of annual training as required. The record contained documentation of approximately 9 of the 16 hours required. 2. In an interview with Caregiver #2 she reported she has completed the required 16 hours of annual training for 2018-2019 but that she has not submitted documentation for approximately 7 hours of completed child credential development classes.

Plan of Correction: The provider will obtain training documentation from Caregiver #2 for the record.
