Comments:
An unannounced renewal inspection was conducted on 10/31/2019. The inspection began at 8:00 AM and concluded at 10:30 AM. There were six children in care with two caregivers and a total of sixteen points. During the inspection, children were observed during free play and engaging with the caregivers. The caregivers were also observed during bottle feeding and hygiene practices. Additionally, the inspector reviewed five children's records, household members' records, and all caregiver records. The inspector examined rooms used for care and the emergency supplies. This provider did not have medications and does not transport children. There was discussion about children's records, provider records, and the renewal process. Violations were reviewed at the conclusion of the inspection. Description: Based on record review and discussion with the provider, the provider failed to obtain documentation of a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance.

Evidence:

1. Child 5 had no documentation of a physical examination and the child's first day of attendance was 07/01/2019. When the LI asked the provider about the physical examination, the provider confirmed that there was no document for the child.
2. Child 3 had no documentation of a physical examination and the child's first day of attendance was 12/04/2017. When the When the LI asked the provider about the physical examination, the provider confirmed that there was no document for the child.

Description: Based on record review, the provider failed to ensure that each report shall include the date of the physical examination and dates immunizations were received and shall be signed by a licensed physician, the physician's designee, or an official of a local health department.
