Technical Assistance:
Continued to remind the provider that all occupied cribs must be spaced at least 12 inches apart. The cribs were not occupied during this inspection.

The Medication Policy statement shall be updated to accurately reflect the fact that the provider's MAT certification has expired.

Comments:
An unannounced monitoring inspection was conducted today. The provider and two assistant caregivers were caring for 10 children, totaling 29 points. Children's records, the physical plant, emergency preparedness items, children's nutrition, caregiver training documentation, caregiver:child interactions, child care programming, and caregiver records were inspected today.

The children were observed playing outside, playing with toys, singing to music, and eating lunch. The children were served cheese pizza, bananas, and milk for lunch today.

Violations:
Standard #: 22VAC40-111-960-E
Description: Based on observations, it was determined that two bottles were not labeled with the child's name.
Evidence:
There was more than one infant in care during this inspection. Once child's bottles were not labeled with the child's name.

Plan of Correction: The bottles will be labeled.

Standard #: 22VAC40-111-60-B
Description: Based on a review of children's records, it was determined that one record does not contain all required information.
Evidence:
Child record D does not include documentation to show that the parent has completed an annual review of the information. This file was most recently reviewed by the parent on 7/9/18.

Plan of Correction: I will ask the parent to review the registration form.

Standard #: 22VAC40-111-210-A
Description: Based on a review of provider training documents, it was determined that the provider does not currently hold satisfactory completion of First Aid and CPR training.
Evidence:
The provider's First Aid and CPR certification expired on 9/23/19.

Plan of Correction: I am scheduled to attend a training tomorrow. I showed the Inspector my enrollment verification during the inspection. My two assistants are here every day and both of them are current with their First Aid and CPR training.

Standard #: 22VAC40-111-240-A
Description: Based on observations, it was determined that one area of the home has not been maintained in a fully clean and operational manner.
Evidence:
The diaper changing pad has ripped in two places.

Plan of Correction: I always place a cover over the pad when doing diaper changes, then clean the diaper pad as required. I will buy a new diaper pad this weekend.
