Comments:
An unannounced monitoring inspection was conducted today from 9:30am to 11:00am with the provider. There were six children in care at the beginning of the inspection, ranging from 19 months to four years of age, for a total of 13 points. Another child, who is three years old, arrived during the inspection. Children were observed eating breakfast, which consisted of oatmeal with cranberries and milk, washing hands, reading books, playing with Play-doh, and having free play. Five children?s records, the provider's record, and one household members? record were reviewed. The provider is current in CPR and First Aid training. There is currently no medication on site, however, the provider is current in Medication Administration Training (MAT). The first aid kit, smoke detectors, and fire extinguisher were observed. The emergency drill log, attendance, and menu were also reviewed. Description: Based on review of five children's records, the provider did not ensure that they reviewed with the parent the emergency contact information annually for each child and obtained the parent's signature acknowledging the review. Evidence: The last documented reviews with parent signature are as follows: for Child #1, 12/18/15; for Child #3, 2/11/17; for Child #4, 5/19/17; and Child #2, with a start date of 12/7/17, does not have documentation of an annual review with parent signature.

Plan of Correction: I will make sure to have the parents review the information and sign right away.
