Comments:
An unannounced monitoring inspection was conducted today from 12:05pm to 1:50pm with the provider. There were four children in care ranging from 14 months to three years of age for a total of 11 points. Children were observed in free play together, doing puzzles, playing with the provider, and washing hands for a snack because they ate an early lunch. Four children?s files, the provider's file, and two household members? files were reviewed. The provider is certified in CPR and First Aid. The provider does not give medication. The first aid kit and fire extinguisher were observed. The emergency drill log, attendance, and menu were also reviewed. Standard #: 22VAC40-191-40-D-4-a
Description: Based on review of two household member's files, the provider did not obtain a sworn disclosure or affirmation when a person age 18 years or older begins residing in the home. Evidence: The records for Household members #1 and #2 did not have documentation of a sworn statement or affirmation. The provider stated that both household members moved in approximately two years ago.

Plan of Correction: The provider stated that she was unaware that this was needed and she will get it completed this week.
