Comments:

Thank you for your time and assistance with the unannounced monitoring inspection conducted on November 1, 2019. The on-site inspection was conducted from 1:10 p.m. to 3:10 p.m. On this date, there were seven children in the care of the provider (16 Points) at the time of arrival. The assistant arrived shortly after to help with the care. There were a total of 13 children enrolled in the program. There were five children's records reviewed. The records of the provider and one assistant/household member were reviewed. There were no medications being administered which is the policy of the provider. Both the provider and the assistant were interviewed. The exceptions to compliance were noted on the violation notice and the risk ratings for the violation were noted on the supplemental page. Some items reviewed and observed were: nap time, free play, hand washing procedures, diaper changing procedures, afternoon snack, outdoor play, emergency evacuation, shelter-in-place and lock down drills, required posted information, daily attendance record, warm water temperature, refrigerator and freezer temperatures, first aid kit and emergency supplies.. Standard #:	22VAC40-111-920
Description:	Based on observation and discussion with the provider, there was not a weekly menu posted for the parents to see what meals and snacks the provider was serving for the current week. Evidence: 1. There was not a posted menu of the food being served for the week of October 28 - November 1, 2019. 2. The provider confirmed that she was posting the daily menu but not posting menus for an entire week.
Plan of Correction:	The provider will ensure that an updated weekly menu is posted for the parents.
Standard #:	22VAC40-111-350-E
Description:	Based on measurements taken and verification from the assistant, the temperature of the hot water exceeded the upper limit of 120 degrees Fahrenheit. Evidence: 1. The temperature of the hot water in the bathroom faucet used by the children measured to be 138.9 degrees Fahrenheit. 2. The assistant measured the hot water from the same faucet with a different thermometer and it registered to be approximately 138 degrees Fahrenheit.
Plan of Correction:	The assistant turned down the hot water heater during the inspection. The provider will check the hot water temperature periodically to ensure it does not exceed 120 degrees.
