Comments:
An unannounced renewal inspection was conducted today between the hours of 10:15am through 11:30am with the provider on site. There were 3 children (7 points) with the provider. A sample of 3 children's files and 2 staff/household member's files were reviewed. The children were engaged in the following: toy play, playing dress up, playing games with the provider, singing and dancing to music. Interactions between the children and provider were positive. The physical space, evacuation drills, attendance records and the Emergency Preparedness Plan were reviewed. Areas of non-compliance are identified in the violation notice. Description: Based on review of children's records, the provider failed to review with the parent the emergency contact information required in 22 VAC 40-111-60 B 2 to ensure the information is correct and obtain the parent's signed acknowledgment of the review.
Evidence:
1. Child #3 began with the provider on Septeber 4, 2018. The parent signed the emergency contact information sheet on 9/04/2018 and did not have documentation of the annual review.

Plan of Correction: I will ask the parent to review and sign today.
