Technical Assistance:
Discussion was held on the topics of record keeping, hand washing, maintenance and household members.

Comments:
A Subsidy Health and Safety inspection was conducted today approximately from 11:00am-3:00pm to review supplemental health and safety requirements. A total of 10 children [30 points] were present with 2 caregivers. A sample of 4 children's records, 1 household member record, 3 caregiver records, the physical space, attendance records, cribs, required posted documents, the Emergency Preparedness Plan, emergency drills and emergency supplies were reviewed. There were no medications at site on this date. Infants were observed crawling, playing with appropriate toys, bottle feeding, and napping. Children were observed solving puzzles, working with phonics flashcards, eating lunch and napping. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection and are identified on the violation notice. Description: Based on document review, the written attendance did not document arrivals and departures of children as it occurred.
Evidence:
1) At the time of inspection, 10 children were in care in the Family Day Home. The attendance sheet documented 8 children. Child #1 and Child #2's names were not documented but were in attendance.
2) Attendance records for the current week and the week before did not document the time of arrival and departures of the children in the FDH.

Plan of Correction: I will provide clear times as to when the arrivals and departures of the children occur.
