Comments:
An unannounced renewal inspection was conducted today from approximately 11:50 pm-1:00 pm. There were 4 children (8 points) in the direct care and supervision of 1 caregiver. A sample of 3 children?s records, 1 caregiver's records and 1 household member's record were reviewed. The children were observed playing with toys, engaged with the provider, eating lunch and washing hands. Zero new caregivers have been hired since the last inspection. Areas of non-compliance are identified in this report. The exit interview was conducted with the provider. If you have any questions, contact Kimberly.Sawyer@dss.virginia.gov.

Standard #: 22VAC40-111-780-B
Description: Based on record review, the provider did not review with the parent the emergency contact information to ensure the information is correct.

Evidence:
The record of Child A was last reviewed 08/13/2018.

Plan of Correction: The parent will complete it today.

Standard #: 22VAC40-111-800-B
Description: Based on observation, the provider did not review the emergency plan at least annually and update the plan as needed.

Evidence:
The emergency plan was last reviewed 04/15/2018.

Plan of Correction: Corrected during inspection.

Standard #: 22VAC40-111-830-A
Description: Based on record review, emergency evacuation procedures were not practiced monthly.

Evidence:
1. Emergency evacuation procedures were not practiced in May of 2019.
2. The facility was open May 1st through the 4th.

Plan of Correction: I was on vacation for the month of May.

Standard #: 22VAC40-111-830-C
Description: Based on record review, documentation of the emergency evacuation did not include required information.

Evidence:
The emergency evacuation documentation for 04/09/2019 was missing the following:
1. Identity of the person conducting the drill.
2. Time of the drill.
3. The method used for notification of the drill.
4. The number of caregivers participating.
5. The number of children participating.
6. Any special conditions simulated.
7. The time it took to complete the drill.
8. Problems encountered, if any.
9. Weather conditions.

Plan of Correction: I will complete it today.
