Comments:
The inspector conducted an unannounced renewal inspection from 10:00 a.m. until 1:00 p.m. There were five children in care for a total of 12 points. Five children's records and two caregiver records were reviewed. Children were observed playing, eating lunch, and during rest time and a diaper change No medication are being administered. Violations had been corrected from the last inspection and violations are identified on the violation notice. Please contact your inspector t 276-698-9981 with any questions.

Violations:
Standard #: 22VAC40-111-830-A
Description: Based on record review, the caregiver did not practice the required amount of drills.
Evidence:
1. The inspector reviewed emergency drills, a drill was not completed in October and the provide acknowledged it was not documented.

Plan of Correction: Caregiver will document monthly drills.

Standard #: 22VAC40-111-830-B
Description: Based on interview, the provider did not have shelter-in-place drills documented.
Evidence:
1. The inspector asked to see the shelter-in-place drills and the caregiver stated she did not have them.

Plan of Correction: Provider stated she would complete two by the end of the year.

Standard #: 22VAC40-111-70-A
Description: The provide failed to provide in writing to the parents required information.
Evidence:
1. The inspector reviewed 5 records. Two of five records did not have the emergency preparedness plan signed by the parents (#1, #4) and #1 did not have the the liability insurance declaration form.

Plan of Correction: Provider will get the information from the parent.

Standard #: 22VAC40-111-90-A
Description: The provider failed to get immunization records before a child's first day of attendance.
Evidence:
1. The inspector reviewed five children's records. One of five records (#1) did not contain an immunization record.

Plan of Correction: The provider will get the immunization record from the parents.

Standard #: 22VAC40-111-90-C
Description: The provider failed to get immunization updates.
Evidence:
1. The inspector reviewed five children's records. Two of five records (# 2, last updated 5/17, and #3 last updated 5/16) did not have immunization updates

Plan of Correction: The provider will get the updates from the parents.

Standard #: 22VAC40-111-180-A
Description: Based on record review, the caregiver did not have an up to date TB screening.
Evidence:
1. The inspector reviewed two records. One of two records (
#1) did not have an up to date TB screening.

Plan of Correction: Caregiver will get the up to date TB screening.

Standard #: 22VAC40-111-230-A
Description: Based on record review and interview, the caretaker did not have written documentation of 16 hrs. of training.
Evidence:
1. The inspector reviewed two records, and certificates were not available for review. The provider had written names of training and the hours but no certificates were available.

Plan of Correction: Provider will get documentation of trainings.

Standard #: 22VAC40-111-330-A
Description: Based on interview, the home did not have a landline.
Evidence:
1. The inspector asked if the provider had a landline and she said no.

Plan of Correction: Provider's have two cell phones. Family member has a landline who lives within 2 miles. May submit paperwork for a variance.
