Comments:

A Subsidy Health and Safety inspection was conducted on 11/7/19 from 11:45am - 1:35pm. At the time of the inspection there were seven children in care, ages four months old to four years old (18 points), with two caregivers present. Records were reviewed for five children and five caregivers. Children were observed eating lunch and during rest period. There was no medication at the family day home currently. Emergency plan and emergency supplies were reviewed. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice.

Description:	Based on observation and interviews, it was determined that the vendor did not ensure that a written hard copy record of daily attendance that documents the arrival and departure of each child in care as it occurs is maintained. Evidence: 1. The attendance record for the family day home did not have the time of arrival for all the children present. 2. Caregiver #1 confirmed that the time of arrival for all the children present was not documented.
Plan of Correction:	The vendor responded: Going forward we will document on the attendance sheet when the children arrive at the family day home immediately.
Standard #:	22VAC40-665-230-E-1
Description:	Based a review of three caregiver records, it was determined that the vendor did not ensure that all staff have a current certification in cardiopulmonary resuscitation (CPR) appropriate to the age of children in care. Evidence: 1. The record for caregiver #2 contained documentation for a certification in cardiopulmonary resuscitation (CPR) appropriate to the age of children in care that expired on 11/6/19. 2. The record for caregiver #3 contained documentation for a certification in cardiopulmonary resuscitation (CPR) appropriate to the age of children in care that expired on 11/3/19. 3. Caregiver #1 confirmed that caregiver #2 and caregiver #3 did not have a current certification in cardiopulmonary resuscitation (CPR) appropriate to the age of children in care.
Plan of Correction:	The vendor responded: Caregiver #2 will complete a course to obtain certification in cardiopulmonary resuscitation (CPR) appropriate to the age of children in care on 11/8/19. Caregiver #3 will complete a course later this month. All new staff will complete a course to obtain certification in cardiopulmonary resuscitation (CPR) appropriate to the age of children in care within 30 days of employment.

Description:	Based on a review of the family day home's policies and interviews, it was determined that the vendor did not ensure that they shall develop and implement the continuity of operations procedures to ensure that essential functions are maintained during an emergency. Evidence: 1. There was no documentation available for viewing during the inspection regarding continuity of operations procedures to ensure that essential functions are maintained during an emergency. 2. Caregiver #1 confirmed that there was no documentation available for viewing during the inspection regarding continuity of operations procedures to ensure that essential functions are maintained during an emergency.
Plan of Correction:	The vendor responded: We will develop procedures for the continuity of operations procedures to ensure that essential functions are maintained during an emergency. A copy of the policy and procedures will be kept at the family day home, and the caregivers will be trained on it.
