Technical Assistance:
Discussion was held on the topics of current enrollment, current staffing, and the renewal process. Since the last inspection, a new assistant has been hired.

Comments:
The following inspection took place between the hours of 8:30 am through 11:30 am. Today, I met with the Provider on site at the Family Day Home (FDH) located in Chantilly, Virginia. At the start of this unannounced renewal inspection there were a total of 3 children observed (10 points) with 1 caregiver. A total of 6 children?s files, 2 staff files, and 3 household member?s files were reviewed. The children were engaged in the following: infant sleeping, diapering, hand washing, infant feeding, toy play, music, song, center play, and other organized activities. Standard #: 22VAC40-111-120-B
Description: Based on review, 1 of 2 staff files indicated that the documentation that two (2) or more references as to character and reputation as well as competency of each staff person was not checked prior to employment.

Evidence -
1. On the date of inspection (11/13/2019), at 0 of 2 references were on file for Staff #2.
2. The start date for Staff #2 was listed as 10/01/2019.

Plan of Correction: References shall be recorded and placed on file for Staff #2.

Standard #: 22VAC40-111-170-B
Description: Based on review, 1 of 2 staff files indicated that TB documentation obtained had not been completed within the last 30 days from their start date.

Evidence -
1. On the date of inspection (11/13/2019), TB documentation obtained for Staff #2 was observed with a completion date of 03/17/2018.
2. The start date of Staff #2 was listed as - 10/01/2019.

Plan of Correction: Updated TB documentation shall be obtained and placed on file for Staff #2.
