Technical Assistance:
Discussion was held on the topics of current enrollment, current staffing, and the status of the renewal application. The Provider stated that the application has been mailed. Discussion was also held on the topic of a substitute provider. All substitute providers must have the same qualifications and all background checks would need to be in place (just as originally required for the actual Provider). Reference checks, Sworn Disclosure Statement, Orientation Training, and CPS & First Aid certifications would also need to be documented and on file with in the Family Day Home.

Comments:
The following inspection took place between the hours of 10 am through 12 pm. Today, I met with the Provider on site at the Family Day Home (FDH) located in Falls Church, Virginia. At the start of this unannounced renewal inspection there were no children in care for today. A total of 1 child?s file, 1 staff file, and 7 household member?s files were reviewed. All previous violation were corrected except for one from the last inspection conducted on October 22, 2019. Violations:
Standard #: 22VAC40-111-830-A
Description: Based on review, emergency evacuation procedures were not practiced monthly with the caregiver and child in care.

Evidence -
1. On the date of inspection (11/15/2019), it was determined that a fire drill for the month of October 2019 was not conducted.
2. The Provider confirmed this observation on the date of inspection.

Plan of Correction: Fire drills shall be conducted at least once every month. Shelter-in-place drills shall be conducted at least once every six months. All drills shall be documented; recording all necessary information. A fire drill shall be conducted tomorrow for November.

Standard #: 22VAC40-111-60-B
Description: Based on review, 1 child record indicated that not all of the required information/documentation was contained within each child's file.

Evidence -
1. On the date of inspection (11/15/2019), the immunization record for Child #1, as required by 22 VAC 40-111-90, was not available for review. This missing documentation was previously cited during an inspection on 10/22/2019.
2. The results of the health examination for Child #1, as required by 22 VAC 40-111-100, was not available for review. The start date for Child #1 was listed as 10/15/2019.

Plan of Correction: The Provider has been informed that the doctor for Child #1 has gone on vacation. This documentation (immunization page and physical) shall be obtain upon the doctor's return.
