Technical Assistance:
Technical Assistance was offered regarding: background checks for new employees, annual trainings, Field Trip permission forms for children.

Comments:
An unannounced monitoring inspection was conducted from 10:25 am through 12:30 pm. There were 6 daycare children and 1 resident child (24 points) present with 2 caregivers present. A total of 5 children's records, 2 caregiver's records, and 2 household member records reviewed. The children were observed playing wit toys, using the bathroom and watching a children's program on television. Infants were observed sleeping in cribs. The emergency drill records were reviewed. Standard #: 22VAC40-111-800-B
Description: Based on record review, the provider did not review the emergency plan at least annually.
Evidence: The emergency plan for the family day home was last updated on 6/12/18. An annual update was not completed.

Plan of Correction: The plan will be updated.

Standard #: 22VAC40-111-180-A
Description: Based on record review, the provider did not obtain for each adult household member a current report of Tuberculosis (TB) Screening form every two years.
Evidence: The TB screening for Household Member # 1 expired on 11/15/19.

Plan of Correction: An updated TB will be obtained.

Standard #: 22VAC40-111-280
Description: Based on observation, potentially poisonous materials were not stored in a locked cabinet, making them accessible to children.
Evidence: There were 2 large bottles of cleaning agents in the unlocked cabinet under the bathroom sink, making them accessible to the children in care. This bathroom is the is used by the daycare children.

Plan of Correction: The cabinet was locked during the inspection.
