Technical Assistance:
Discussed the role of a volunteer and the background requirements for all volunteers.

Comments:
An unannounced monitoring inspection was conducted this date. A total of 11 children (35pts), one of which was the Provider's own child with Provider and 1 Assistant and 1 Volunteer. When the Inspector arrived, the children were seated and getting ready for morning snack. Morning snack was gold fish and sip cups. 2 infants were sleeping in the crib room, and one staff observed holding a third infant while bottle feeding. The home was organized and well maintained. Topical ointments, medications and emergency drills were reviewed along with 6 children's records, both staff and one volunteer record were reviewed. One child's birthday was being celebrated today. Children free played and danced and sang to music. Hand washing observed prior to lunch. While lunch was being prepared, staff read books to the children.

Violations:
Standard #: 22VAC40-111-700-A
Description: Based on review of medication, one medication authorization was not obtained.
Evidence: Child #4 has a medication on site, no parent written medication authorization was on file.

Plan of Correction: I will have this parent complete the authorization for 10 working days.

Standard #: 22VAC40-111-700-B
Description: Based on review of medications and child records, parent's written authorizations for medications were not renewed after 10 working days..
Evidence: Child #1 and #3's parent written authorization for medication exceeded ten working days. Authorizations for medication must be either renewed by the parent after 10 working days or a written physician authorization must be obtained.

Plan of Correction: I will have both parents sign another written authorization for 10 days only. If parents would like the medication to be maintained longer, a doctor authorization will be obtained.

Standard #: 22VAC40-111-750-D
Description: Based on review of topical ointments, one topical ointment was expired.
Evidence: Child #6 has a topical ointment that expired in 9/2019.

Plan of Correction: I will return this ointment to the parent.
