Discussed obtaining documentation for new crib purchase, and trampoline usage during operating hours.

A mandated Renewal inspection was conducted today. There were 12 children present (34 points) with 3 caregivers. Children were observed coloring Thanksgiving pictures, playing with musical instruments, playing with fire trucks and cars, reading books, playing with babies, and having diaper changed. Reviewed 2 children's files, 3 staff files, and 4 household members files, emergency evacuation drill,menu, crib compliance, and attendance. There were no medications on site. Evidence:
1. Child#1's record was missing the name, address and phone number of 2 Emergency contacts, first date of attendance, and child's insurance policy number.Plan of Correction: I will obtain the information from the parent.
Standard #: 22VAC40-111-70-A Description: Based on record review, parent's were not given required information regarding the family day care home's policies.
Evidence:
Child#1 and Child#2 did not have documentation of parent receipt of liability insurance, emergency plan, and medication policy.Plan of Correction: I will give this information to the parent
