One household member will be turning 18 years of age before the next inspection. The provider was reminded of the background check requirements that will need to be completed once the household member turns 18.

Today an unannounced Monitoring inspection took place between 9:55am and 11:15am. There were 9 children present with the provider and 1 assistant for a total of 23 points within the supervision guidelines. The children were observed watching cartoons, playing with wooden blocks and building towers, doing activity books, singing and dancing. A complete inspection of the physical plant, children and staff records, fire drill log, menu, attendance and emergency procedures were observed during this inspection. Standard #: 22VAC40-111-90-A Description: Based on record review and interview, the provider did not obtain updated immunizations once every six months for children under the age of two years.
Evidence: Child #5 (23 months old), most recent immunizations are dated 1/17/2019. The provider confirmed that the families has not provided more current immunization records.Plan of Correction: I'm sure she's received the shots and I'll request the record.
