Comments:
An unannounced monitoring inspection was conducted from 10:15 a.m. - 11:45 a.m. There were ten children in care with the provider and one assistant, ages 3 months, 5 months, 8 months, 11 months, 20 months, 2 years, 2 years, 2 years, 3 years, and 4 years for a total of 29 points. The children were observed watching television, participating in free play, and eating lunch. Ten children's records were reviewed and in compliance. The provider and one assistant records were reviewed and in compliance. There are reportedly no medications being administered to children in the family day home at this time. Violations:
Standard #: 22VAC40-111-830-A
Description: Based on observation of the drill log and interview with the provider it was determined that emergency evacuation drills were not practiced monthly. Evidence: The most recent emergency evacuation drill was conducted 06/03/2019.

Plan of Correction: The provider will practice emergency evacuation drills every month.

Standard #: 22VAC40-111-830-B
Description: Based on observation of the drill log and interview with the provider it was determined that shelter-in-place drills were not practiced at a minimum of twice per year. Evidence: There were no shelter-in-place drills documented for the year of 2019 on the drill log.

Plan of Correction: The provider will practice shelter-in-place drills at least twice per year.

Standard #: 22VAC40-111-960-E
Description: Based on observation of bottles it was determined that not all bottles were labeled with children's full name and date. Evidence: Two bottles were observed in the refrigerator without the child's full name and date.

Plan of Correction: The provider will ensure that all bottles are labeled with the child's full name and date.

Standard #: 22VAC40-111-50-A
Description: Based on observation and interview with the provider it was determined that a written record of attendance was not kept daily. Evidence: There was no written documentation of attendance for the week of November 11, 2019 - November 13, 2019.

Plan of Correction: The provider will complete attendance daily.

Standard #: 22VAC40-111-530-F
Description: Based on observation of the nap-room it was determined that soft bedding such as pillows was not prohibited for infants. Evidence: Child A (dob 7/24/2019) was observed in their crib laying on a wedge pillow.

Plan of Correction: The provider removed the pillow and returned it to the parent of child A during the inspection.

Standard #: 22VAC40-111-610-B
Description: Based on observation it was determined that children were not offered activities while watching television. Evidence: The children were observed sitting in high-chairs facing the television for approximately 30 minutes without additional activities available to them. After 30 minutes when some of the children began to cry the children were removed from the high-chairs and the television was turned off.

Plan of Correction: Children will be offered activities while watching television.
