Comments:
An unannounced monitoring inspection was completed on 11/20/2019. There were four children in care with the provider and a household member/assistant supervising at a total of nine points. Four children's records, the provider's record and one household member's (assistant) record were reviewed. The inspector arrived at the provider's home at 1:20 P.M. and departed at 2:45 P.M.

The children were observed during free play times, nap and TV time.

There was discussion with the provider about the following: annual training, fire drill happening at different time of day and emergency contacts.

Violations:
Standard #: 22VAC40-111-180-A
Description: Based on review of the records, the provider failed to ensure that all repeat TB screenings were completed every two years.

Evidence:.
1. Provider's and household member's most recent TB screening and or test was 10/6/2017

Plan of Correction: The provider and house hold member will go Monday and will monitor all time frames.
