Comments:
Today an unannounced Renewal inspection took place between 8:20am and 11:55am. There were 6 children present with the provider and 1 assistant for a total of 14 points within the supervision guidelines. The children were observed being dropped off by their parents, eating breakfast (cereal and apple slices), doing puzzles, looking at books, and playing outside. A complete inspection of the physical plant, children and staff records, fire drill log, menu, attendance and emergency procedures were observed during this inspection. Violations:
Standard #: 22VAC40-111-760-A-1
Description: Based on observation, the first aid kid did not contain minimum supplies.
Evidence: The first aid kit does not contain tweezers or a digital thermometer.

Plan of Correction: We took them out. I will replace them.

Standard #: 22VAC40-111-770
Description: Based on observation and interview, the provider does not have a working battery-operated flashlight or a working portable battery-operated weather band radio available.
Evidence: The flashlight and battery operated radio that are available do not work. The provider stated that they might need new batteries.

Plan of Correction: I will replace the batteries tonight.

Standard #: 22VAC40-111-120-A
Description: Based on record review, staff records did not contain all required information.
Evidence: Caregiver #3 file did not contain documentation of the first date of employment, an emergency contact to be reached in case of an emergency or two reference checks.

Plan of Correction: I will see her on Thanksgiving and she can fill it out.

Standard #: 22VAC40-111-60-B
Description: Based on record review, children's records did not contain all required information.
Evidence: Child #2 file does not contain documentation of an annual review of emergency contact information. Child #3 file does not contain documentation of the street address of their first emergency contact.

Plan of Correction: I'll send them the missing information.

Standard #: 22VAC40-111-80-B
Description: Based on record review, the provider did not verify the identity and age of a child within the first seven days of attendance.
Evidence: Child #1 file (date of enrollment: 5/24/19) file did not contain documentation that the child's identity and age has been verified.

Plan of Correction: They brought it and I forgot to fill out the form.

Standard #: 22VAC40-191-40-D-5-a
Description: Based on record review, the provider has not obtained a central registry background check for each household member over the age of 14 years old.
Evidence: Household member #2 (14 years old) and household member #3 (15 years old) files do not contain documentation of a central registry background check. This was previously cited at the provider's last inspection on 8/5/2019. The provider provided documentation to the inspector that the central registry background checks had been filled out on 8/19/2019.

Plan of Correction: The provider contacted the office of background investigations (OBI) during today's inspection. OBI stated that they send the provider the results on 9/4/2019. The provider will print the results and add them to the household member files.
