Comments:
An unannounced monitoring inspection was conducted on 12/03/2019 from 1:40 P.M.-3:45 P.M. At the time of inspection, four children (ten points) were in the care of two caregivers. Five children's records, one provider record, one caregiver record, one pet vaccine, evacuation drills, first aid kit, emergency supplies, emergency plan, required posted information, and outside play area were reviewed. Children were observed during naptime, waking up from naptime, infant interactions, and preparation of snack time. The exceptions to compliance are noted on the violation notice.

Standard #: 22VAC40-111-100-A
Description: Based on review of children files, the center failed to obtain a physical exam prior to child's attendance or within 30 days of enrollment.

Evidence:
1. Child #3's file did not contain documentation of a physical exam. Date of enrollment for Child #3 was 01/08/2019.
2.The provider verified that there was no documentation of a physical exam in the child's file.

Plan of Correction: Provider will talk with the guardian and request a copy of a physical.
