Standard #: 22VAC40-111-60-A Description: Based on interview with the provider, it was determined that the provider did not maintan a record for each child in care. Evidence: Child #3, age 5 months, did not have a file. Plan of Correction: A file will be created for Child #3, my granddaughter.
Standard #: 22VAC40-111-560-A Description: Based on observation, it was determined that a caregiver was not providing direct care and supervision including awareness and responsibiility of each child and being near enough to intervene. Evidence: A 5 month old child was observed on the couch in the back of the childcare area near the fridge. The provider answered the door and the assistant was observed in the bathroom assisting another child. The 5 month old was in the back of the room, on laying on the couch approximately 20 feet from the front door and around the corner from the bathroom out of sight of caregivers. Plan of Correction: The child is my granddaughter and I was not considering her a daycare child. In the future, all daycare requirements will be followed for all children.
