Technical Assistance:
Due to the background check related violation cited in today's inspection, the Provider is required to follow-up with the appropriate department regarding the status of any missing and/or incomplete background checks. Documentation of the completed follow-up must be sent to the Licensing Inspector within 10 business days, and a copy of all missing/incomplete background check results must be sent to the Licensing Inspector upon arrival.

Comments:
An unannounced monitoring inspection was conducted from 12:15pm-1:45pm. There were 6 children (12 points) in the direct care and supervision of the Provider and the Assistant Provider. A sample size of 3 children's records, 2 caregiver records, evacuation drills, the first aid kit, and the daycare space were reviewed. The children were observed setting the table for lunch, eating lunch (tortillas, ground beef, salsa, lettuce, cheese, refried beans, fruit, and milk), using the restroom, and laying down for their designated rest period. Description: Based on a review of caregiver records, one caregiver did not have a central registry finding within 30 days of employment.
Evidence:
1. Caregiver #2 (doh: 5/18/2019) did not have documentation of a central registry finding on file during the inspection.

Plan of Correction: I sent it out, but it was returned because I need to use the current form. I will send it back out by Friday.
