Comments:
An unannounced renewal inspection was conducted on December 2, 2019 from approximately 11:00 AM to 1:45 PM to determine the provider's compliance with licensing standards. In attendance were 5 children, totaling 15 points, being supervised by the provider. The children were observed interacting with the provider, watching a movie, washing hands, having lunch, and napping. Lunch today consisted of ham, green beans, pineapple, bread and milk. Children's books, toys, and equipment were inspected and found to be age appropriate and in operable condition. The children's restroom was inspected with the required supplies present. The family day home had the required postings displayed. The first aid kit and emergency supplies were inspected with the required items found. The emergency preparedness plan was reviewed. There is no medication being administered at this time. Five children's records and the provider's records were inspected. The following was also inspected: emergency drill logs, parent agreements, and other pertinent information. Standard #: 22VAC40-111-830-A
Description: Based on record review and interview, the provider did not ensure that emergency evacuation procedures shall be practiced monthly. Evidence: The licensing inspector observed the emergency evacuation practice log indicated no evacuation drill was practiced in September 2019 or October 2019. The provider reported the evacuation drills had not been conducted in September or October 2019.

Plan of Correction: The provider will ensure that emergency evacuation drills will be conducted and recorded each month.

Standard #: 22VAC40-111-920
Description: Based on observation and interview, the provider did not ensure that the posted menu shall list any substituted food. Evidence: The licensing inspector observed the posted menu listed the day's lunch as chicken nuggets, peas, apple juice and a roll. Ham, green beans, pineapple, bread and milk were served for lunch during the inspection.

Plan of Correction: The provider will ensure that any substituted food is listed on the posted menu.

Standard #: 22VAC40-111-100-A
Description: Based on a review of five children's records, one record did not contain documentation of a physical examination by or under the direction of a physician either prior to the child's enrollment or within 30 days after the first day of attendance. Evidence: The record for Child #3 (start date 9/30/19) did not contain documentation of a physical examination.

Plan of Correction: The provider will request the physical exam from the child's parent.

Standard #: 22VAC40-111-120-B
Description: Based on a review two caregiver records, one record did not contain the required documentation. Evidence: The record for Caregiver #2 (estimated start date 10/15/19) did not contain the job title, and start date.

Plan of Correction: The provider will update the caregiver record.

Standard #: 22VAC40-111-60-A
Description: Based on a review of five children's records, five records did not contain the required information. Evidence: 1. The licensing inspector observed the record for Child #1 (start date 9/25/19) did not contain the policy number for medical insurance, a second emergency contact, or information on who can pick up the child. 2. The records for Child #2 (start date 9/20/19) and Child #3 (start date 9/30/19) did not contain policy numbers for medical insurance. 3. The record for Child #4 (start date 10/1/19) did not contain a policy number for medical insurance or an address for the second emergency contact. 4. The record for Child #5 (start date 10/1/19) did not contain a policy number for medical insurance or a second emergency contact.

Plan of Correction: The provider will speak to parents and request the required information for the children's records.

Standard #: 22VAC40-111-200-A
Description: Based on the review of two caregiver records and interview, the assistant did not complete orientation training by the end of the first week of assuming job responsibilities. Evidence: The licensing inspector observed the record for Caregiver #2 (estimated start date 10/15/19) did not contain documentation of a completed orientation training. The provider reported some, but not all, of the required orientation training items had been completed.

Plan of Correction: The provider will ensure the orientation is completed and added to the caregiver record.
