Comments:
The licensing inspector conducted an unannounced monitoring inspection, at which play groups, morning snack time, activities, and games were observed. Children were observed; and staff and children files and other documentation were reviewed. Two (2) children, 4 years of age, were found to be in care totaling 4 points. Previous violations were reviewed, and had been corrected. Please contact me if you need further assistance. Thank you for your cooperation. This inspection began at 8:30 am and concluded at 10:15 am.

Violations:
Standard #: 22VAC40-111-780-B
Description: Based on review of children records the provider failed to maintain compliance with this standard as required related to annual verification of information by the parents.

Evidence:
Children records #1-3, enrolled as of 4/1/18, did not have annual parental verification of emergency information as required.

Plan of Correction: All records will be annually reviewed and signed off on by the parents as required; any updates or changes will also be made at this time.

Standard #: 22VAC40-111-100-A
Description: Based on review of child records the provider failed to maintain compliance with this standard as required related to documentation of physicals.

Evidence:
Child records #1 and #2, enrollment date 4/1/18, did not have a physical as required.

Plan of Correction: The parent was notified and a copy of the most recent physical for each child, signed and dated by the physician, was requested.

Standard #: 22VAC40-111-60-A
Description: Based on review of children records the provider failed to maintain compliance with this standard as required relating to contact information and verification of birth.

Evidence:
Child record #1, enrollment date 4/01/18, did not have the following items as required:
father's place of employment, work address, work phone number, the mother's work address, work phone number, and neither of the emergency contacts had addresses.
Child record #2, enrollment date 4/01/18, did not have the following items as required:
Child record #4, enrollment date 8/31/18, did not have a physical address, the father's work address, work phone number, and physical address were not addressed, and the mothers place of employment, address, and phone number were also not addressed.

Children records #1, 2, 4, and 5 (enrollment date 8/2019) did not have proof of birth adequately recorded as required. Not all of the information needed was addressed.

Plan of Correction: Records have already been reviewed and updated information has been requested from the parents. The information is slowly being returned and will be documented and filed upon receipt.

Standard #: 22VAC40-111-90-A
Description: Based on review of child records the provider failed to maintain compliance with this standard as required related to proof of immunization.

Evidence:
Child record #1, enrollment date 4/1/18, did not have proof of immunizations as required.

Plan of Correction: The parent was notified and a copy of the most recent immunization record, signed and dated by the physician, was requested.
