The licensing inspector conducted an unannounced monitoring inspection, at which play groups, activities, group time, story time, personal hygiene, and lunch time were observed. Children were observed; and staff and children files and other documentation were reviewed. Ten (10) children, aged 8 months-3 years, were found to be in care totaling 24 points with the provider and assistant. Please contact me if you need further assistance. Standard #: 22VAC40-111-700-B Description: Based on discussions with the provider and review of medications the provider failed to maintain compliance with this standard as required relating to authorization to administer medications.
The authorization of medication for child B had expired as of 9/9/19. The medication was still in the medication box at the time of inspection.
Plan of Correction: The parent will be notified again as to the need to pick up the medication or have a physician fill out a long term use authorization to administer form. If the medication is not picked up within 2 weeks, the medication will be disposed of.
Standard #: 22VAC40-111-710 Description: Based on discussions with the provider and review of medications the provider failed to maintain compliance with this standard as required relating to medications being in the original labeled pharmacy container.
The medication for Child B did not have the pharmacy prescription label attached and was not in the pharmacy issued container.
Plan of Correction: The parents will be notified immediately of the need for a pharmacy label, or the need to bring the original box with the label attached.
Standard #: 22VAC40-111-920 Description: Based on discussions with the provider and review of items posted the provider failed to maintain compliance with this standard as required related to weekly menu.
There was no menu posted or available for the week as required.
Plan of Correction: The menu will be posted by the end of the day and a new menu will be posted prior to opening each Monday as required.
Standard #: 22VAC40-111-180-A Description: Based on discussions with the provider and review of records the provider failed to maintain compliance with this standard as required relating to renewal of PPD tests.
Staff file #1 did not have a renewed PPD test or screening as required. The one on file had expired as of 11/14/2019.
Plan of Correction: A new PPD test or screening will be obtained as oon as possible and placed in the file for future compliance.
