Comments:
An unannounced monitoring inspection was conducted on 12/3/19 from 9:25 am until 11:30 pm at Wonderland Daycare, LLC. At the time of the inspection nine children were in care with the provider and a caregiver. The children and caregivers were observed during indoor free play, bathroom break, diapering, hand washing, arrival, snack, bottle feeding, transitions and behavioral management. Violations were found during this inspection and are documented on the violation notice. If you have questions or concerns contact the licensing inspector at (540) 848-4123 for further assistance.

Violations:
Standard #: 22VAC40-111-100-A
Description: Based on record review an interview, the provider failed to obtain documentation of a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance.

Evidence:

1. The records for five children were reviewed.
Child 3's start date was listed as 6/25/19. Child 3's record did not contain documentation of a physical.
Child 5's start date was listed as 10/12/19. Child 5's record did not contain documentation of a physical.
2. The provider verified she does not have documentation of a physical for child 3 or child 5.

Plan of Correction: The parents of child 3 and child 4 will be advised to provide documentation of a physical or the children will not be allowed to return. In the future if documentation of a physical is not provided by the 30th day of care the child will no longer be allowed to return until the documentation is provided.

Standard #: 22VAC40-111-60-B
Description: Based on record review and interview, the provider failed to obtain all the required information for children's records.

Evidence:

1. The record for child 2 did not contain an address for the emergency contact, a home address and written authorization for emergency medical care.
2. The provider verified she did not have this information for child 2.

Plan of Correction: The missing information will be obtained for child 2. In the future all paperwork will be reviewed upon receipt to ensure all required information is obtained.

Standard #: 22VAC40-111-210-A
Description: Based on record review and interview, the provider and caretaker 2 failed to obtain a minimum of 16 annual training hours annually.

Evidence:

1. The record of the provider (time frame 11/19/18-11/18/19) and caregiver 2 (time frame 3/26/18-3/2519) did not contain documentation of annual training.
2. The provider stated they completed training but she did not have documentation.

Plan of Correction: Both the provider and caretaker 2 will complete the required training hours next year. The provider will keep documentation of the training in the staff records.
