Technical Assistance:
Safe sleep practices were discussed with the provider during the inspection.

Comments:
An unannounced monitoring inspection was conducted on 12/10/19 from 1:00pm until 1:50pm. At the time of entrance, there were 5 children in care, for a total of 16 points, with the provider. A sample of 5 children's records and 2 provider/household member records were reviewed. Children were observed resting quietly during nap time and engaging in free play. The provider does not currently administer medication and there have been no injuries to children in care since the last inspection. First aid and emergency supplies, documentation of emergency practice drills, the emergency preparedness plan and required family day home postings were reviewed. Two violations in the areas of administration are identified on the violation notice and were discussed with the provider during the exit interview.

Violations:
Standard #: 22VAC40-111-100-A
Description: Based on record review and interview, the provider did not obtain documentation of a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance.

Evidence:
1. The record for child 1 (date of enrollment: 9/4/19) does not contain documentation of a physical examination.
2. The provider confirmed that child 1's record is lacking documentation of a physical examination.

Plan of Correction: The provider will contact the parent of child 1 and request a copy of the most recent physical examination, which will be placed in the child's record.

Standard #: 22VAC40-111-60-B
Description: Based on record review and interview, the provider did not ensure that children's records contain all the required information.

Evidence:
1. The record for child 2 does not contain the address of one of the two designated persons to contact in case of an emergency if the parent cannot be reached.
2. The provider confirmed that the record for child 2 is lacking an address for one of the two emergency contacts.

Plan of Correction: The provider obtained the address for the second emergency contact from child 2's parent during the inspection and documented it in the child's record.
