Technical Assistance:
Central Registry Search requirements for household members over the age of 14 years was discussed with the provider during the inspection.

Comments:
An unannounced monitoring inspection was conducted on 12/10/19 from 2:20pm until 3:15pm. At the time of entrance, there were 9 children in care, for a total of 18 points, with the assistant caregiver. The provider arrived to the home during the course of the inspection. A sample of 5 children's records and 3 provider/household member/assistant records were reviewed. Children were observed resting quietly during nap time and eating snack. Restroom, hand washing and food service procedures were also observed. First aid and emergency supplies, the emergency preparedness plan, documentation of emergency practice drills and and required family day home postings were reviewed. The provider stated there are currently no children in care that require medication and there have been no accidents or injuries since the last inspection. Three violations in the areas of administration and care of children are identified on the violation notice and were discussed with the provider during the exit interview.

Violations:
Standard #: 22VAC40-111-60-B
Description: Based on record review and interview, the provider did not ensure that children's records contain all the required information.

Evidence:
1. The record for child 2 (date of enrollment: 9/3/19) does not contain the address of one of the two designated persons to contact in case of an emergency if the parent cannot be reached.
2. The provider confirmed that the record for child 2 is lacking an address for one of the two emergency contacts.

Plan of Correction: The provider will contact child 2's parent to obtain the missing address for the emergency contact person.

Standard #: 22VAC40-111-90-A
Description: Based on record review and interview, the provider did not ensure that documentation that the child has been adequately immunized was obtained before the cild may attend the family day home.

Evidence:
1. The record for child 1 (date of enrollment: 10/1/19) does not contain documentation of immunizations.
2. The provider confirmed that the record for child 1 is lacking documentation of immunizations.

Plan of Correction: The provider will contact the parent of child 1 and request a copy of the most recent immunization record.

Standard #: 22VAC40-111-570-A
Description: Based on observation and record review, the provider did not ensure that a caregiver does not exceed 16 points.

Evidence:
Upon the inspector's arrival to the home at 2:20pm, the assistant caregiver was alone with 9 children, who were napping. The children ranged in age from 2 years to 4 years, for a total of 18 points. The assistant caregiver reported that the provider had left the home to run an errand. The provider returned to the home at approximately 2:40pm.

Plan of Correction: The provider will ensure that each caregiver does not exceed 16 points at any time.
