Technical Assistance:

We discussed the requirements for immunization record updates and annual training. The risk assessment for a violation was provided. We reviewed the requirements for children's records, emergency response plan updates/reviews, and nap equipment.

Comments:

Thank you for your assistance during the unannounced monitoring inspection conducted from 12:43 PM to 4:00 PM. Today, there were five children, ages infancy through three years (fourteen points), with the provider. I viewed daily routines, activities, staffing, supervision, interactions with children, equipment, posted information, five records for children, two caregiver/resident records, caregiver training, emergency plan, emergency supplies and drill records, nutrition/menu, diapering procedures, indoor and outdoor areas. The provider was observed providing individual interactions with the children and attended to needs for care and attention. Regular outside experiences have been enjoyed, weather permitting. The provider has taken children on neighborhood walks. There have not been any minor injuries to treat. No transportation provided. Let me know if you need any assistance. 540/430-9259

Standard #:	22VAC40-111-90-C
Description:	Based on a review of five records for children, and an interview with the provider, the provider did not obtain documentation of additional immunizations for a child at least once every six months under the age of two years. Evidence: Child 4 will not be two years in age until November 2020 and the most recent immunization report was dated April 10, 2019.
Plan of Correction:	The child just had another doctor visit. Another signed immunization record will be obtained from the parent.
Standard #:	22VAC40-111-210-A
Description:	Based on a review of documented annual training, and an interview with the provider, the provider did not obtain at least sixteen hours of annual training in areas relevant to the job responsibilities. Evidence: There was written information for the completion of ten and one-half hours of training by October 2019. The provider indicated she is working on another on-line training this month and thought the hours were due at the end of 2019.
Plan of Correction:	The provider will make sure she completes at least sixteen hours of training by November 14, 2020. Additional sources for training on-line will be considered.
