An unannounced monitoring inspection was conducted on 12/10/2019 from 12:05 pm - 12:50 pm. At the time of entrance there were four children in care with the provider. The ages of the children counted for a total of eight points. Children were observed during nap. The entire home and backyard play area was reviewed. Records reviewed for one new child and the provider.
The violations are listed on the violation notice and were discussed with the provider at the conclusion of the inspection.

Standard #: 22VAC40-111-100-A
Description: Based on record review, the provider failed to obtain documentation of a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance.

1. Child 1 lacked a documented physical exam on file.
a. Child 1 has an enrollment date of 09/30/2019 and was in care during the inspection.
2. A parent discharge paper was on file for child 1 however, this does not meet the requirement of a physical examination.

Plan of Correction: The parent of child 1 will be asked to provide documentation of a current physical exam within 30 days.

Standard #: 22VAC40-111-70-A
Description: Based on record review, the provider failed to ensure before the child's first day of attendance, parents shall be provided in writing policies for the administration of medications.

The record for child 1 did not include written medicaiton policies for the family day home.

Plan of Correction: The provider stated that she verbally stated to the parent of child 1 that medication is not administered.
A written medication form will be given to the parent to indicate no medication is administered by the provider.
