Comments:
An unannounced monitoring inspection was conducted from 11:15am through 12:40pm. There were six children in care ages five months through four years old. Points for children in care totaled 17. The provider and one assistant caregiver were present during the inspection. was not present at the onset of the inspection. Children were observed finishing a craft, eating lunch, restroom routines, transitions and nap. Children, caregiver and household records were reviewed as well as required postings, emergency drills and emergency items. The provider stated that there is not any medication being administered nor are children in care transported. Violations in the areas of children's records and emergency evacuation drills were identified and appear on the violation notice. These violations were reviewed with the provider at the conclusion of the inspection.

Violations:
Standard #: 22VAC40-111-830-A
Description: Based on record review and interview the provider failed to ensure that emergency evacuation was practiced each month as required.
Evidence:
1 - The log sheet documenting emergency evacuations practiced does not show that a drill was held during the month of November.
2 - The provider confirmed that there was not a drill practiced in November.

Plan of Correction: Going forward the provider will ensure that a drill is practiced each month.

Standard #: 22VAC40-111-60-B
Description: Based on record review the provider failed to ensure that each child's record contains all required information.
Evidence:
1 - The record for child 1 does not contain documentation of annual update for the 2019 calendar year.
2 - The record for child 2 does not contain documentation that information regarding liability insurance for the home was provided to the parent.

Plan of Correction: The information will be obtained from and provided to the parents as required.
