Comments:
The licensing inspector conducted an unannounced renewal inspection from 9:30 am - 11:30 am. There were nine children in care for a total of twenty seven points with two caregivers. The children were observed playing with blocks, listening to a story, singing, and at free play. Three caregiver's records and five children's records were reviewed. Violations from the previous inspection were also reviewed and one was repeated. Areas of non-compliance are identified on the violation notice. Violations:
Standard #: 22VAC40-111-240-A
Description: Based on observation, the provider failed to maintain the outside play area in a safe condition.
Evidence:
1. The inspector observed the outside play area. There is black plastic protruding through the mulch under the swings creating tripping hazards. There is a plastic picnic table that has cracks in the seat area that could pinch skin. There is a plastic blue car that has cracks in the plastic.

Plan of Correction: The provider has been approved for grant money that will be used to replace the picnic table and any broken toys. The picnic table and car will be removed from the play area. Mulch will be raked into the areas under the swings before the children play outside again.
