Comments:
An unannounced monitoring inspection was conducted today from 10:30am-12:00pm. There were 9 children (26 points) supervised by 3 direct care staff. The home, 2 staff records, 5 children?s records, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today. Children were observed playing outside and eating lunch. Lunch served was chicken, rice, mixed vegetables, grapes, apples, and milk. Handwashing procedures were also observed. There was a sufficient amount of books, toys, and materials, which were found to be developmentally appropriate for the children in care at the time of inspection. Areas of non-compliance are identified in the Violation Notice.

Standard #: 22VAC40-111-960-E
Description: Based on observation, the provider did not ensure that all bottles were labeled with the child's full name and date.

Evidence: Child #1 (age: 6 months old) had 2 breastmilk bottle bags that were not labeled with the date.

Plan of Correction: Corrected during inspection.

Standard #: 22VAC40-111-60-B
Description: Based on record review, 2 of 5 children's records did not contain documentation of all required information.

Evidence:
1. Child #1's (age: 6 months old) record did not contain documentation of parental acknowledgement of receipt of the emergency preparedness plan.
2. Child #2's (age: 7 months old) record did not contain documentation of proof of identification and a complete address for the second emergency contact person.

Plan of Correction: I will contact the parents today to request missing information.

Standard #: 22VAC40-191-60-C-2
Description: Based on record review, 1 of 2 staff records did not contain documentation of central registry findings within 30 days of employment.

Evidence: Staff A's (date of hire: 9/2019) record did not contain documentation of central registry findings.

Plan of Correction: I will send the request today.
