Comments:
An unannounced monitoring inspection was conducted today from 12:45pm-2:00pm. There were 11 children (29 points) supervised by 2 direct care staff. The home, 2 staff records, 6 children?s records, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today. Children were observed watching television while transitioning to naptime and playing a fishing game. There was a sufficient amount of books, toys, and materials, which were found to be developmentally appropriate for the children in care at the time of inspection. Areas of non-compliance are identified in the Violation Notice.

Standard #: 22VAC40-111-60-B
Description: Based on record review, 3 of 6 children's records did not contain documentation of all required information.

Evidence:
1. Child #3's (age: 3 years old) record did not contain documentation of a complete address for both emergency contact persons, and a completed physical signed by a physician.
2. Child #4's (age: 3 years old) record did not contain documentation of the name and address of each parents' place of employment.
3. Child #5's (age: 9 months old) record did not contain documentation of a completed immunization record signed by a physician.

Plan of Correction: I will ask the parents for the missing information today.
